A renal transplant recipient who had atypical clinical manifestations, unclear epidemiological exposure history and negative results from influenza virus antigen and nucleic acid amplification in throat swab specimens was admitted into our hospital on April 17, 2013. I have no idea what made my usual primary care doctor so sure visually it was herpes, not conduct a swab and to go by a positive result of HSV 1 by blood test and negative for HSV 2 and only to be told weeks later my swab/cultures from being poked in 4 different genital lesions were negative for herpes 1 and 2. The only test that can be done is to take a biopsy (quick snippet of skin) from the suspect bump and do a histology section on it to determine if it is wart tissue or inflammatory. If the confirmatory state lab comes back negative for HSV, then the case is dismissed and the physicians may end the antiviral treatment. The presenting problem at that time was described as recurrent impetigo of the left pinna.
